Service Agreement between Pet Parent(s) and Hilltop Pet Resorts


Vaccinations
I understand that pets, like humans, are susceptible to various strains of viruses and other illnesses. I also understand that because there are many strains of viruses, vaccinations are not always effective. I understand that much depends on a pet’s immune system, exposure to other pets, age, stress and nutritional status. I also understand that Hilltop Pet Resorts and Salon, Inc. requires written records from your veterinarian of Bordetella every 12 months and current rabies and DHLPP+C (distemper combo) for all boarding and daycare pets.
I understand that I must present written documentation of current immunizations and agree to ADVANTAGE application or I will not be permitted to leave my pet at the facility. The only exception is if I agree to have the vaccinations administered to my pet(s) during their stay at the facility.

Advantage Flea and Tick
To protect my pet(s) from fleas, lice, ticks and/or other parasites, I agree to application of ADVANTAGE Topical Solution. Initial application will be upon arrival for all boarding and daycare pets. Advantage application has no contraindications and is effective within 12 hours. Reapplication will be on a monthly basis. The only exception to this policy will be if Advantage has been applied within one week prior to arrival at Hilltop Pet Resorts or the pet is less than 7 weeks old and over 5 pounds. Hilltop Pet Resorts will track application and reapplication dates’, thus assuring each pet is protected at all times. Any evidence of infestation will be reported to the pet parent, along with instructions on follow-up care. A fee for this service will be included on the invoice on departure.

Medical Treatment in the Pet Parent’s Absence
In my absence, and after all reasonable efforts to reach me and/or the emergency contact I have designated have been made, I hereby give my permission to Hilltop Pet Resorts and Salon, Inc. to have care provided by one of the following depending upon Hilltop Pet Resorts assessment of the situation and to act as my agent in case of an emergency or in the case of any apparent health issue regarding my pet(s). My pet’s usual Veterinary clinic/hospitalOrSalt Lake Veterinary Services, a mobile veterinary service operated by Dr. Rick Whitty who will come to Hilltop Pet Resorts and Salon to care for my pet(s)OrAdvanced Veterinary Care (AVC), 1021 E 3300 S Salt Lake City, UT, which is open and staffed with a veterinarian 24 hours a day every day of the year.

I agree to reimburse Hilltop Pet Resorts and Salon, Inc. for charges incurred for any veterinary care, diagnostic test(s), treatment and/or medications including ADVANTAGE provided to my pet (s).


Regarding Pet Safety
Pets are supervised at all times. The outside yards are enclosed in 8-foot fences. Unless you alert us in writing that your pet may be unsafe in this environment we will assume our fencing is sufficient to safely enclose your pet(s).

Dogs displaying aggressive behavior will be confined to their playpen and isolated when outside during their current stay. This also applies to pets who jump the inside doors and/or the 4-foot fences between the yards. Upon the owner’s return, owner(s) will be informed that for safety reasons the pet cannot return to Hilltop Pet Resorts and Salon, Inc. in the future.
Pet diets will be fed as per the instructions provided by Pet Parent(s) to Hilltop Pet Resorts and Salon, Inc. Pet Parents must provide food for any special dietary needs; otherwise pet(s) are fed a high quality kibble with gravy twice a day. Pet Parents must provide food and/or treats to be given during daycare or boarding.

Medications and/or treatments are administered as per the instructions provided by the Pet Parent’s Veterinarian. Pet Parents must provide any medication and/or treatment supplies in original packaging with written instruction from the Veterinarian. If Pet Parent’s instructions are unclear or contradict the written Veterinarian instructions, Little Dog’s Resort will call veterinarian to confirm instructions. If Pet Parent brings unlabeled medication or treatment supplies, Pet Parent will be required to sign an additional Unlabeled Medication Waiver and Little Dog’s Resort will confirm with your Veterinarian prior to administering any medications or treatment supplies.

Only Pet Parent(s) or previously designated person whose name has been provided to Hilltop Pet Resorts and Salon staff in advance may pick up pets from Hilltop Pet Resorts and Salon. I understand that if I or a person I have designated have not picked up my pet(s) from boarding or daycare by closing time, my pet will be fed and put to bed for the night. I may then pick up my pet(s) the next day during regular business hours. I will be charged the standard overnight boarding fee for the night.

I understand that Hilltop Pet Resorts and Salon, Inc. makes every reasonable effort to provide a safe and secure place for my pet to receive boarding, grooming and daycare services. Under the conditions described above, I agree that I will not hold Hilltop Pet Resorts and Salon, Inc. responsible should an accident, allergic reaction or illness occur while Hilltop Pet Resorts and Salon, Inc. is providing reasonable and safe care for my pet. I also understand that Hilltop Pet Resorts and Salon, Inc. is not responsible for lost or misplaced items that I bring with my pet (bowls, blankets, toy, collars, shampoo, shot records etc).


Refusal of Service
I understand that Hilltop Pet Resorts has the right to refuse service to me and/or my pet at any time for any reason. I understand that if my pet has a history of or repeatedly demonstrates aggression or biting of humans or animals, Hilltop Pet Resorts reserves the right to refuse service. I understand that all bites resulting in a workman’s compensation claim will be reported to the local authorities as required by law.
I hereby declare to Hilltop Pet Resorts that I am the legal owner of my pet.


Media Release
I hereby give to Hilltop Pet Resorts and Salon, Inc. its heirs, legal representatives and assigns, those for whom the photographer is acting, and those acting with Hilltop Pet Resorts and Salon Inc’s authority and permission:
a) the unrestricted right and permission to copyright and use, re-use, publish, and republish photographic portraits or pictures of me or my pet(s) in which I may be included intact or in part, composite or distorted in character or form, without restriction as to changes or transformations in conjunction with my own or a fictitious name, or reproduction hereof in color or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade, or any other purpose whatsoever.
b) I also permit the use of any printed material in connection therewith.
c) I hereby relinquish any right that I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.
d) I hereby release, discharge and agree to save harmless Hilltop Pet Resorts and Salon Inc, its heirs, legal representatives or assigns, and all persons functioning under his/her permission or authority, or those for whom he/she is functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.
e) I hereby permit Hilltop Pet Resorts and Salon, Inc. to publish any and all media aforementioned to social media and website platforms such as Instagram, Facebook, etc. and to utilize “tagging” “liking” or otherwise identify me in connection with photographic or video content of me or my pet(s).

Cancellation, No Show, and Deposit Policy
Holiday Cancellation – Cancellations for holiday boarding reservations made 6 weeks prior to check in will receive a 100% refund. Cancellations made 4 weeks prior to check-in will receive a 50% refund. Any cancellation received within 4 weeks of are non-refundable and non-transferable. This includes your $50 deposit.
Non Holiday – Cancellations for boarding reservations made 4 weeks prior to check in will receive a 100% refund. Cancellations made 2 weeks prior to check-in will receive a 50% refund. Any cancellation received within 2 weeks of are non-refundable and non-transferable. This includes your $50 deposit.
No Show – If you do not cancel your reservation and do not show up, you will be billed a cancellation fee equal to 100% of the amount of your reservation. This includes your $50 deposit.
Deposits are credited to your account and deducted from services provided.

Packages, Memberships & Punch Cards – These services are non-refundable and non-transferable.

Release and Waiver of Liability
I hereby release Hilltop Pet Resorts and Salon, Inc., its agents, officers, sub-contractors, employees, animal owners, customers, and potential customers of Hilltop Pet Resorts and Salon, Inc. from any and all liabilities, financial, and otherwise, for injuries to myself, my pet, or any other property of mine, which arise in any way from services and/or products provided by or as a consequence of my association with Hilltop Pet Resorts and Salon, Inc.

 

Reservations
